Office hours

  • Documents are submitted only by e-mail. In special cases, it is possible to arrange (via e-mail) a meeting to provide documents in person.
  • The office hours take place on Thursdays (on days with Thursday schedule) from 12.15pm – 1.45pm in a remote form via the MS Teams application. Office hours in the faculty building are canceled.

Internship options

At the Faculty of Mathematics and Information Science you can get credits for the student internship in three different ways:

Work, mandate contracts or being entrepreneur
Internship regulations allow your work contract or being an enterpreneur to credit as student internship. In order to pass the internship based on such contract you have to prove that the scope of your work is compatible with your field of studies. Each case of passing the internship in this way is considered individually. Work performed abroad can also credit as an internship. All documents can be in English, for documents in other languages official translation into Polish or English is required. Required documents:

  1. Application for crediting internships based on a work contract [DOC, PDFENG, DOC, PDFPL]

    Attention: please do not enter the survey as an attachment.

  2. Copy of the work contract or certificate of entry in the business activity register
  3. Internship program (signed by the employer prior to the start of the internship period).

    Attention: there is no editable model of the program, the employer should list the intern’s duties (which prove the compliance with the study program).

  4. Report on the course of the internship (signed by the employer after the end of the internship period) [DOC, PDFENG, DOC, PDFPL].
  5. Confirmation of the end of internship (signed by the employer after the end of the internship period) [DOC, PDFENG, DOC, PDFPL].
  6. Survey for the employer [XLS – all fields of study at the Faculty (editable version), PDF – Computer Science and Information Systems]

    Attention: an employer should be provided with the questionnaire appropriate for the given field of study.
    Attention: the survey is for statistical purposes, it does not replace the certificate of duties.

Internships in companies
(on the basis of agreement between the University and company)
Attention: The agreement between the University and company has to be signed before the beginning of the internship. Documents submitted at a later date will not be considered. There is no legal possibility for the Warsaw University of Technology to sign an agreement with a company established outside of Poland. Required documents (all documents must be filled in Polish):

  1. Internship program (program must be accepted by the Dean’s Proxy and signed by employer)
  2. Proof of Accident Effects Insurance Attention: it can be (although it not obligatory) an insurance offered by the Warsaw University of Technology, details on the website (in Polish).
  3. Internship agreement between the company and the Faculty [DOC, PDF] and the necessary annexes [PDF] Attention: three copies are required, each copy must be signed by the employer and the student, Dean’s Proxy will sign the documents only after all beforementioned documents are provided.
  4. Referral for internship and Certificate of completing the internship [DOC, PDF] Attention: only one copy of the referral and certificate is required; filled and signed referral should be forwarded to the employer, only filled and signed certificate should be returned to the Faculty.
  5. Internship report [DOC, PDF] Attention: only one copy of the report is required. You need to rewrite the following learning outcomes for Computer Science and Information Systems field of study:

    1. Has knowledge of how to realize IT projects and processes.
    2. Performs tasks in a project or IT process using methods and technologies specified by his/her superiors.
    3. Cooperates within a team and/or with customer’s representative.
  6. Survey for the employer [XLS – all fields of study at the Faculty (editable version), PDF – Computer Science and Information Systems] Attention: an employer should be provided with the questionnaire appropriate for the given field of study
  7. Invitation for the employer to register at the university’s employers database (the paper version should be picked up together with the signed agreement and referral)
Internal internships
Internship regulations allow you to take internships at the Faculty’s research and teaching units. Internal internship is possible under the care of one of the Faculty’s employees. Employees must submit the application (to Vice-Dean for Student Affairs) for approval. Please consult the details with internship regulations or Dean’s Proxy for Professional Training Placement. Required documents:

  1. Application for the internal internship, scope of work and employee’s opinion (after the internship) (filled by the employee/teacher) [DOC, PDF] You need to rewrite the following learning outcomes for Computer Science and Information Systems field of study:

    1. Has knowledge of how to realize IT projects and processes.
    2. Performs tasks in a project or IT process using methods and technologies specified by his/her superiors.
